You can instantly have peace of mind because we use "Clear Care." With the Family Room Portal, log in from anywhere  on any device, to see real-time what was done in the home and which caregiver was there, find out if there was a change in condition, and even review invoices.

"Change in Condition" reporting, every shift

Our new Quality of Care module, further empowers us to improve outcomes and lower health care spending by monitoring, reporting, and preventing serious complications among clients with chronic conditions.

The "Quality of Care" reporting module consists of two main components:

Change in Condition protocol, developed as part of a multi-year study with Harvard Medical School, provides transparency into clients' health by identifying changes in a client’s condition and monitoring (or helping to manage) these changes in the home.

Hospitalization Tracking gives us visibility into client admissions providing for more seamless continuity of care and transitions back to the home. Tracking over time helps to identify trends, making it possible to take proactive action to reduce readmissions.

This results in better care for our shared clients and reduced hospitalizations.

All Care staff, are required to comment on well-being of client, prior to leaving their home. 

An RN or LPN reviews this information and updates families if required.

How Do We Support Our Employees and Health Care Providers?


Nursing Support 24/7

We provide 24/7 Nursing Support for  caregivers on duty. We know how anxiety-provoking it can be to feel unprepared for changes in  clients condition while  caregivers are on duty. 

There will always be a Nursing Supervisor's cell number, reachable 24/7 for urgent concerns with clients. 

Smart Phone App

We provide caregivers with a Clear Care app for their smart /android phone upon hiring.

Caregivers need to arrive to shifts feeling prepared. Not only do they need to easily see where  they’re supposed to be and when,  but tjey also need to know critical details about the shift like tasks and specific client needs. With ClearCare’s " Clear Care Go" mobile app, caregivers can log in from anywhere to access driving directions via Google Maps and daily, weekly, or monthly shift details.
